Hypofractionated stereotactic radiotherapy for large brain metastases: Optimizing the dosimetric parameters

Stereotactic radiotherapy plays a major role in the treatment of brain metastases (BM). We aimed to compare dosimetric results four plans for hypofractionated stereotactic (HFSRT) large metastases. Ten patients treated with upfront NovalisTx® non-coplanar multiple dynamic conformal arcs (DCA) HFSRT ≥ 25 mm diameter single BM were included. Three other volumetric modulated arc therapy (VMAT) evaluated: coplanar (Eclipse®, Varian, VMATcEclipse®), and (VMATncEclipse®), (Elements Cranial SRS®, Brainlab, VMATncElements®). The marginal dose prescribed PTV was 23.1 Gy (isodose 70%) three fractions. mean GTV 27 mm3. Better conformity indices found all VMAT techniques compared DCA (1.05 vs 1.28, P < 0.05). gradient VMATncElements® (2.43 3.02, 0.001).
